Welcome to UCLA Orthopaedic SurgeryThank you for choosing UCLA for your orthopaedic care. Our staff would like to make your experience with our office a pleasurable one.  Please bring your insurance card and photo identification to every visit.

Please dress comfortably. The doctor will need to examine the area that is bothering you. You may also need to get an x-ray and wearing shorts, a short sleeve blouse, etc. may allow you the chance to not have to put on an examination gown.

If you have had an x-ray, scan or a MRI it is very important that you bring film or CD with you to the appointment. It is also very important that you bring the written interpretation with you. The doctor will want to see both of these items. If you are being referred by another physician please bring all pertinent medical records, film and reports.

Important: If your insurance plan requires a referral, please bring it with you. 

It is our policy to collect all co-pays and fees not covered by your insurance at the time of the visit. If you do not have insurance, full payment is expected at the time of the visit. We accept personal check, MasterCard, Visa, American Express and Discover.  We will bill your insurance provider for Professional Service Fees.
